Top Surgery is the most commonly performed gender-affirmation/gender-reassignment surgery for transgender and non-binary patients – it is an important step in the transition journey. Top Surgery is a reconstructive surgical procedure tailored to the unique desired surgical outcome for the individual and is designed to provide the necessary changes to the chest and breasts for patients who are interested in transforming their bodies towards the gender with which they identify, that being male, female, or neither.

Female to Male (FTM) / Female to Non-Binary (FTN) Top Surgery: Chest masculinizing Top Surgery removes the feminine characteristics of the breasts and reconstructs the chest towards a more masculinized phenotype (a “flat” chest).

For FTM/N, the surgical goals are to reduce “female” characteristics and make the chest look more flat and chiseled. FTM/N Top Surgery involves a specialized type of breast reduction which typically includes removing a significant portion of breast tissue and excess skin, positioning of symmetric scars, altering the size and location of the nipple and areola, and using liposuction to sculpt and perfect the chest contour. This is performed in a manner to preserve as much sensation as possible, and minimize scarring when possible. There are a variety of surgical approaches to FTM/N Top Surgery (such as Double Incision or Periareolar), and the specific approach for each patient is tailored to their individual anatomy. Aspects of the post-surgical result such as the degree of chest flatness or nipple-areola position can be adjusted to the patient’s desired surgical outcome. We do offer the option of completely removing the nipples and areolae for those patients who feel these anatomic structures are incongruent with their identity.

Male to Female (MTF) / Male to Non-Binary (MTN) Top Surgery: With MTF/N breast augmentation surgery, our board certified Top Surgery specialist plastic surgeons use techniques to create pockets under the breast in which breast implants will be placed to create the size and volume that the patient desires.

There are a variety of surgical approaches to MTF/N Top Surgery (such as incision location, breast implant size & shape, and breast implant position relative to the pectoralis muscle), and the specific approach for each patient will be tailored to their individual anatomy and desired surgical outcome. This is performed in a manner to preserve as much breast sensation as possible and to minimize scarring.
